Consent for PAN Verification

I/We hereby give my/our consent to Jana Small Finance Bank Limited ("the Bank") for conducting PAN verification, as per following terms and conditions:

• I/We hereby declare that all the information is voluntarily furnished by me/us and the same is true, correct and complete.
• I/we will not hold the Bank or any of its official responsible for any incorrect information provided by me/us.
• I/We have voluntarily opted for PAN verification and have submitted to the Bank my/our PAN number, Name as per PAN and Date of Birth.
• I/We now expressly consent that the Bank has the right to validate my/our PAN information as needed for present or future requirements.
• The contents of this consent and purpose of collecting information has been explained to me/us in my/our local language.
• I/We understand that for PAN verification, the Bank will share PAN number and/or Name and/or Date of Birth with National Securities Depository Limited ("NSDL"), and NSDL will share with the Bank, PAN verification data, Date of Birth Match status, Name Match status, PAN- Aadhaar link status, which shall be used for providing services to me/us.
• I/We confirm that this consent shall remain valid as long as I/we maintain an account with the Bank or avail any service from the Bank.
